UroFirm™ is GentsMed's name for the standardized intracavernosal botulinum toxin protocol. Botulinum toxin (Botox, Dysport, Xeomin, or Letybo) is injected directly into the corpora cavernosa — the erectile tissue of the penis — where it temporarily blocks the sympathetic (adrenergic) nerve signals responsible for keeping the penis in its flaccid state. Reducing that "anti-erection" tone improves erectile response and produces a modest increase in flaccid length.

How it works

Erection is fundamentally a balance: parasympathetic signaling triggers blood flow into the erectile tissue, while sympathetic signaling maintains flaccidity and aborts erection. Botulinum toxin binds at the neuromuscular junction and blocks norepinephrine release from sympathetic nerves into corporal smooth muscle. The result: less sympathetic "brake" opposing erection, easier blood flow, better erection quality. The mechanism is fundamentally different from PDE5 inhibitors (sildenafil, tadalafil), which amplify the pro-erection signal rather than reducing the anti-erection one.

What the evidence shows

Randomized controlled trials of intracavernosal botulinum toxin for ED have demonstrated significant improvements in Erection Hardness Score and IIEF-EF subdomain scores compared to placebo.12 Trials have also documented a measurable increase in stretched penile length of approximately 1.5 cm at six months.3 The evidence base is still developing, but the procedure has moved from experimental to a recognized second-line option in the urology literature.

Who it is for

Most appropriate for men with erectile dysfunction who have only partially responded to PDE5 inhibitors, men who want to reduce or eliminate oral-medication dependence, men with mild-to-moderate vasculogenic ED, and men exploring non-pharmaceutical options before escalating to Trimix or other intracavernosal therapies. UroFirm™ is also reasonable as a complement to GentShot (Penile PRP) — they act through different mechanisms and can stack.

What to expect

  • In-office procedure, ~20–30 minutes total.
  • Typically 100 units of Botox (or equivalent dose of Dysport / Xeomin / Letybo) reconstituted in preservative-free saline.
  • Topical anesthesia + fine-gauge needle. Most patients report mild pressure rather than pain.
  • Same-day return to normal activity. Minor bruising or tenderness possible for 24–48 hours.
  • No noticeable effect in the first 1–10 days. Onset begins ~day 10, builds through month 3, and gradually declines after month 4–5.
  • Repeat treatment at 4–6 month intervals maintains the response.

Important considerations

  • Priapism (erection lasting >4 hours) is the most important risk and requires emergency-room evaluation. The risk is low with the standardized UroFirm™ dose and protocol, but every patient is counseled to recognize and respond appropriately.
  • UroFirm™ is fundamentally different from cosmetic facial Botox. The drug is the same; the injection target, dose, and protocol are completely different. Providers performing this technique require UroFirm™ certification.
  • The effect is temporary by design. If a patient discontinues, normal erectile function returns to its untreated baseline within ~6 months. This is a feature, not a bug — botulinum toxin has the longest real-world safety record of any ED intervention precisely because it is reversible.
  • Not appropriate for severe vasculogenic ED unresponsive to all therapies, men with bleeding disorders or active genital infection, or men with known hypersensitivity to botulinum toxin products.
