1. Information we collect

We collect the information you provide when you fill out forms on the site — including your name, email address, and, when you choose to share it, your phone number and whether you have opted in to text messages. We also collect standard analytics about how the site is used. We do not collect, store, or process Protected Health Information (PHI). Anything medical happens between you and a licensed provider, in their system.

2. How we use it

Submitted information is used to evaluate applications, route inquiries to the appropriate team, and operate the network. We don't sell your information to third parties.

3. Cookies + analytics

The site uses cookies necessary to operate basic functionality and may use privacy-preserving analytics to understand aggregate usage. We don't use tracking cookies for advertising purposes.

4. Third parties

Some pages link to providers' own practice sites and intake systems. Those are operated by the providers themselves and have their own privacy practices.

5. Mobile information & text messaging (SMS)

If you give us your mobile number and opt in, we use it to send you text messages related to your request, appointments, orders, and account, and — only where you separately agree — occasional updates. You can opt out at any time by replying STOP, or reply HELP for help. See our SMS Terms for program details, message frequency, and rates.

No mobile information — including your phone number and your SMS opt-in or consent — will be shared with or sold to third parties or affiliates for their own marketing or promotional purposes. We share mobile information only with the service providers that help us deliver these messages (for example, our messaging platform and customer-relationship system), and only so they can send the texts you asked for on our behalf. Text messaging originator opt-in data and consent are never shared with any third parties for their own use.

6. Your rights

You can request a copy of any information we hold about you, or request its deletion, by emailing [email protected].

7. Changes

We'll update this policy as needed. Substantive changes will be announced via a notice at the top of this page and an updated date.
