Is UroFirm™ the same drug as cosmetic facial Botox?

The drug itself is the same — onabotulinumtoxinA (Botox) or an equivalent such as Dysport, Xeomin, or Letybo. What is completely different is the injection target, dose, depth, and protocol. UroFirm™ is an intracavernosal injection into erectile tissue; facial Botox is an intramuscular injection into facial expression muscles. Because of those differences, providers performing UroFirm™ require specific certification — facial-injection training does not cross over.

How long does the effect last?

Onset is around day 10, peak effect at 3 months, gradual decline after month 4–5, and most of the benefit is gone by month 6. Maintenance injections at 4–6 month intervals keep the response steady. A small subset of patients see longer durability — closer to 8 months — and a small subset see shorter, closer to 4 months. Individual variation in botulinum toxin metabolism is well-documented across all cosmetic and therapeutic applications.

Is it permanent if I keep getting it?

No — botulinum toxin is always temporary. Each dose works for ~6 months and then breaks down completely. There is no cumulative permanent change. Can I combine UroFirm™ with GentShot (Penile PRP)?

Yes — they act through completely different mechanisms (UroFirm reduces sympathetic anti-erection tone; GentShot is regenerative). They are commonly stacked in patients with multifactorial ED. Timing varies by provider preference. Some clinicians do them on the same day; others separate them by 2–4 weeks to assess the response to each independently. What is the most important risk?

Priapism — an erection lasting more than 4 hours — is the rare but serious risk. Untreated priapism can cause permanent tissue damage and worsen long-term ED. With the standardized dose and protocol, the incidence is very low, but the consequence is high enough that every patient must know what to watch for.
