Is UroFirm™ the same drug as cosmetic facial Botox?

The drug itself is the same — onabotulinumtoxinA (Botox) or an equivalent such as Dysport, Xeomin, or Letybo. What is completely different is the injection target, dose, depth, and protocol. UroFirm™ is an intracavernosal injection into erectile tissue; facial Botox is an intramuscular injection into facial expression muscles. Because of those differences, providers performing UroFirm™ require specific certification — facial-injection training does not cross over.

Does it actually treat ED, or is it cosmetic?

Primarily ED. The peer-reviewed randomized controlled trial evidence supporting UroFirm™ is built around ED endpoints — Erection Hardness Score, IIEF-EF improvement, ability to maintain erection for intercourse. The flaccid-state size gain (~1.5 cm stretched length at 6 months) is a documented secondary effect, but the primary indication and the strongest evidence base are functional. For patients whose main goal is cosmetic girth enhancement, the Urosculpt™ penile filler procedure is a better match. The two procedures stack — about half of UroFirm patients in our network also undergo penile filler within 12 months.

How long does the effect last?

Onset is around day 10, peak effect at 3 months, gradual decline after month 4–5, and most of the benefit is gone by month 6. Maintenance injections at 4–6 month intervals keep the response steady. A small subset of patients see longer durability — closer to 8 months — and a small subset see shorter, closer to 4 months. Individual variation in botulinum toxin metabolism is well-documented across all cosmetic and therapeutic applications.

Is it permanent if I keep getting it?

No — botulinum toxin is always temporary. Each dose works for ~6 months and then breaks down completely. There is no cumulative permanent change. If you stop treatment, your erectile function and flaccid baseline return to their pre-treatment state within about 6 months. This reversibility is one of the main reasons UroFirm™ has a favorable safety profile compared to surgical interventions or long-term medication exposure.

Can I combine UroFirm™ with GentShot (Penile PRP)?

Accepted Answer

Yes — they act through completely different mechanisms (UroFirm reduces sympathetic anti-erection tone; GentShot is regenerative). They are commonly stacked in patients with multifactorial ED. Timing varies by provider preference. Some clinicians do them on the same day; others separate them by 2–4 weeks to assess the response to each independently. Your provider will recommend the best sequence for your case.

What is the most important risk?

Priapism — an erection lasting more than 4 hours — is the rare but serious risk. Untreated priapism can cause permanent tissue damage and worsen long-term ED. Every UroFirm™ patient is counseled to recognize the symptoms and go directly to an emergency room if it occurs. With the standardized dose and protocol, the incidence is very low, but the consequence is high enough that every patient must know what to watch for. Common, minor side effects — injection-site tenderness, small bruise, brief soreness — resolve in 24–48 hours.
