How it works

GIP and GLP-1 are gut hormones released after eating. Tirzepatide mimics both. GLP-1 activity drives appetite suppression and slowed gastric emptying; GIP activity contributes additional metabolic effects on adipose tissue and insulin response. The combination produces deeper satiety and larger weight loss outcomes than either mechanism alone.

What the evidence supports

The SURMOUNT trials are the FDA-registration data. SURMOUNT-1 showed average weight loss of ~21% on the maximum 15 mg weekly dose over 72 weeks — substantially more than semaglutide's ~15% over a comparable timeframe in STEP.1 SURMOUNT-2 confirmed similar magnitude in patients with type 2 diabetes.2

Who it is for

Zepbound is FDA-approved for adults with BMI ≥30, or ≥27 with at least one weight-related comorbidity.3 Many clinics also offer compounded tirzepatide off-label at lower BMIs at provider discretion — same caveat as compounded semaglutide.

Side effects and trade-offs

  • Same GI profile as semaglutide: nausea, constipation, reflux — typically dose-dependent and easing with slow titration.
