How it works
GLP-1 (glucagon-like peptide-1) is a gut hormone released after eating. Semaglutide mimics its action, producing satiety with smaller meals and reducing food noise — the constant low-grade urge to snack that drives a lot of overweight. It also improves insulin sensitivity and lowers postprandial glucose spikes.
What the evidence supports
In the STEP trials (the FDA-registration data for Wegovy), patients on the maximum 2.4 mg dose lost an average of ~15% of body weight over 68 weeks — substantially more than any prior oral weight-loss drug.1 STEP 5 extended this to two-year outcomes showing durable loss.3 SELECT later demonstrated a 20% cardiovascular event reduction in overweight patients with established cardiovascular disease.2
Who it is for
Wegovy is FDA-approved for adults with BMI ≥30, or ≥27 with at least one weight-related comorbidity such as hypertension, dyslipidemia, or type 2 diabetes.4 Some clinics offer compounded semaglutide off-label at lower BMIs based on provider judgment — that decision is between you and a physician.
Side effects and trade-offs
- GI symptoms — nausea, constipation, reflux — are common in the titration phase. Most ease with slow dose escalation and dietary adjustment.
- Muscle mass loss is a real concern at higher doses. Protein intake and resistance training are non-optional, not optional.
- Gallbladder events (cholecystitis, gallstones) occur more often than placebo, particularly with rapid weight loss.
- After discontinuation, regain is the default. Successful long-term outcomes pair the drug with sustained lifestyle change.
