Testosterone cypionate is the most prescribed form of testosterone replacement therapy in the United States. It is a long-acting ester of bioidentical testosterone, designed for intramuscular or subcutaneous administration on a weekly (or twice-weekly) schedule. Used correctly, it restores testosterone to physiological ranges and resolves the symptoms of clinically low testosterone in most men.4
How it works
The cypionate ester slows release of testosterone into circulation after injection. A weekly dose typically produces a peak at 48–72 hours and a trough at the end of the week. Many physicians split the weekly dose into two smaller injections to smooth out the curve and improve estradiol management.
Who it is for
TRT is for men with symptoms of low testosterone — low libido, low energy, mood changes, depressed muscle mass and bone density — and a confirmed biochemical diagnosis.12 The standard workup is two morning total testosterone measurements on separate days, with one or both under approximately 300 ng/dL, alongside free testosterone, SHBG, LH, FSH, prolactin, estradiol (sensitive), CBC, CMP, lipid panel, and PSA at baseline.1
TRT is not a longevity drug, not a substitute for sleep or training, and not a tool for healthy-baseline men chasing supraphysiological levels. The patients who benefit most are symptomatic men with confirmed low labs whose providers have ruled out reversible causes first.2
What to expect
Most men report improved energy and libido within 3–6 weeks of starting.3 Body composition changes (muscle gain, fat loss) follow over 3–6 months. Dosing is titrated to symptoms and lab response, not to a fixed number. Quarterly labs are standard after stabilization to monitor hematocrit, estradiol, lipids, and PSA.1
Important considerations
- Fertility: TRT suppresses sperm production. Men wanting to conceive should discuss HCG or alternative protocols before starting.
- Hematocrit: TRT raises red-cell mass. Routine monitoring and occasional phlebotomy may be needed.
- Prostate: TRT does not cause prostate cancer, but it can accelerate growth of pre-existing disease. PSA monitoring is part of standard follow-up.
- Estradiol: managed through dose timing, not reflexive anastrozole. Aggressive estrogen suppression causes more problems than it solves.
